As a fully accredited member of the Australian Natural Therapists Association (ANTA) I support good privacy practice and agree to comply with the privacy legislation in regards to all information received from my patients/clients (hereinafter referred to as “clients”).

Collection of personal information

Clinical Context

In collecting personal information about clients I will ensure that they have contact details for me and my practice; that they can gain access to the information collected about them; that I will only collect information about them which assists me in performing my functions as a practitioner of naturopathy, western herbal medicine and neuro emotional technique; and that I will inform them of any information I collect from a source other than them personally, in compliance with the privacy legislation.

Website

When a client subscribes to my email list I collect their name and email address to send occasional email updates. This information will be used for the specific reasons that you entered them into the forms. For example, to send emails, to contact you for a connection call or to respond to an enquiry or booking.

We receive, collect and store any information you enter on this website or provide us in any other way. In addition, we collect the Internet protocol (IP) address used to connect your computer to the Internet; computer and connection information. We may use software tools to measure and collect session information, including page response times, length of visits to certain pages, page interaction information, and methods used to browse away from the page. 

 

Why is personal information collected?

Non-personal and Personal Information is collected for the following purposes:

  1. To provide and operate the services;

  2. To provide users with support;

  3. To be able to contact visitors and users with general or personalized service-related notices and promotional messages with their permission;

  4. To review how and when this website accessed;

  5. To comply with any applicable laws and regulations.

 

Use of personal information

Clinical Context

I will only use information for the purpose or purposes for which it was collected or as otherwise permitted their personal by law. The main reason for the collection of their personal information by me is to establish an accurate, clear and comprehensive record of each consultation including, but not limited to name, address, contact details and occupation of the client; date of consultation; presenting complaint including duration; past and family history; relevant lifestyle history, details of previous treatments and current medications; known allergic reactions; and findings from pathology tests or other investigations. I will only disclose their personal information for the purpose or purposes of consultation with another practitioner or as otherwise permitted by law and only with the express permission of my client.

This website is hosted on the Wix.com platform. Wix.com provides the online platform that allows for services and products to be offered. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. Wix.com stores your data on secure servers behind a firewall. 

All direct payment gateways offered by Wix.com adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.

How will clients and site visitors be contacted?

Clinical context

Clients may be contacted via email, phone, text messages or post regarding appointments, enquiries, health information, accounts, or legal information.

Website

Website visitors will only be contacted if they fill out a form on this website requesting to be contacted, if they email me or if they subscribe to the email list, or to enforce user agreement or any legal obligations. Website visitors may be contacted if they purchase a product in order to finalise any account details or product delivery or feedback. Subscribers can opt-out of contact by unsubscribing to emails.

Quality and security of data

I will take all reasonable steps to make sure that the personal information I collect is accurate, complete and up-to date. I will take all reasonable steps to ensure that the storage, use and any transfer of their personal information is done in such a way as to best protect their privacy. I will take all reasonable steps to destroy or permanently de-identify personal information if it is no longer needed for any purpose for which the information may be used or disclosed under National Privacy Principle 2.

If you communicate with me via electronic means such as email, Zoom, Skype, Doxy.me, contact forms, Facebook and other internet platforms, I do not have full control over the transmission or storage or any personal information disclosed. You agree that by partaking in such forms of communication you understand and accept there is an intrinsic risk of disclosure or loss of your personal information for which I cannot be held responsible.

Openness

This policy is available to all my clients. On request I can provide further general detail on what sort of personal information I hold, for what purpose, and how I collect, hold and use that information.

 

Access and correction

Any client is entitled at all times to request access to their own information and correct any personal information I hold about them. If access to their information is denied under specified circumstances set out in the privacy legislation, I will provide them with adequate reasons for the denial of access.

 

Identifiers

I will not adopt as my own identifier any identifier that has been assigned to a client by a government agency except where required by law.

 

Anonymity

Wherever it is lawful, practicable and relevant I will ensure my clients have the right to be dealt with anonymously.

Transborder data flows

I will not transfer personal information about clients outside Australia for any purpose without the express permission of my client.

 

Sensitive information

I will not collect sensitive information about clients without their prior consent or as required by law or otherwise as set out in the privacy legislation.
